At Advanced Correctional Healthcare, Inc., employees have two options from United Healthcare - UMR to choose from for medical coverage:

PPO with Embedded Deductible

HSA High Deductible with Embedded Deductible

    • Individual deductible: $3,000

• Plan has a $50 individual and $150 family deductible ($1,000 yearly max)

• 80% Basic Care

• 50% Major

• 50% Ortho

At Advanced Correctional Healthcare, Inc., vision coverage is offered through Vision Service Plan - VSP.com:

• These benefits are provided through VSP – Vision Service Plan.

• This plan does not provide insurance cards, you can log into vsp.com for benefit information.

• Providers just need social security number to verify benefits.

LIFE & SHORT-TERM INSURANCE

  • • Employees can elect coverage amounts up to $500,000 in increments of $5,000.

    • Evidence of Insurability form required for any election over $150,000.

  • • Spouse can elect coverage amounts up to $250,000 in increments of $5,000.

    • Cannot select a coverage greater than 100% of employee elected amount.

    • Evidence of Insurability form required for any election over $50,000.

  • • Coverage amount is $10,000.

    • Employee must be enrolled in Employee Voluntary Term Life Insurance with a minimum of $10,000 elected coverage.

  • • Provides 60% of your earnings, (maximum of $500 per week) for up to 90 days.

    *pre-existing clause if you were treated or diagnosed 3 months prior to effective date or within the first year of plan. Coverage may be denied.

SAVINGS ACCOUNTS

  • • Enables you to contribute up to $3,650 for an individual and $7,300 for a family.

    • Catch-up contribution (age 55+) is $1,000.

  • • Enables you to set aside annual deposits up to $2,850 for expenses, which are not covered or are ineligible for payment through insurance.

At Advanced Correctional Healthcare, Inc., 401(k) plans are operated through ADP.

• Available to employees on the 1st day of the next month after hire date.

• After one year of employment ACH will match 100% of the first 3% and 50% from 3% to 5% of your contribution after one year of employment.

  • • Full-time employees begin accruing vacation time on their start date and can begin taking vacation time as it accrues.

    • Less than five year 40-hour employees accrue 96 hours per year.

    • 5+ year 40-hour employees accrue 136 hours per year.

  • • ACH recognizes six paid holidays per year:

    • New Year’s Day

    • Memorial Day

    • Independence Day

    • Labor Day

    • Thanksgiving Day

    • Christmas Day

    • Hourly employees paid at time and a half if worked within the 24-hour holiday.

  • • Allows employees to work a non-traditional schedule to meet the employee’s personal needs for a period of time.

    • Employees must make up time within the same week.

  • • Full-time employees begin accruing sick time on their start date and can begin taking sick time as it accrues.

    • Employees earn .0231 hours of sick time times the number of hours worked per pay period.

  • • Max of 24 hours (up to three consecutive working days) for employees unable to work due to the death of an immediate family member.

  • • Summoned employees will be excused from work with pay.

    • Payment will be the difference between regular pay and jury pay.

    • (Up to eight hours per day, 40 hours per week, two weeks per year).

  • • Accrual of service time toward seniority, additional paid time off, retirement plan eligibility, and etcetera continues.

    • Upon return, employee will be restored to former job or comparable.

  • • Unpaid leave for employees who have worked at least one year, worked 1,250 hours over the last 12 months, and work in a facility with at least 50 employees within a 75-mile radius can take up to 12 weeks.

    • 26 weeks for employee to care for a covered family member in the line of duty.

  • • Unpaid leave for employees who are not eligible for FMLA.

    • Can take up to twelve weeks.

  • • Employees are recognized for their years of service, quarterly awards, certifications, personal achievements, educational achievements and more.

  • • Referral bonuses are offered for referrals.

    • Bonus will be paid for each candidate referred by a current employee who is hired and remains an employee for at least 30 days.

  • • After six months of service, receive up to $1,500 per year (75% reimbursement of expense) to cover tuition and other fees for job related continuing education.

    • Must have approval from human resources.

    • Employees must remain with ACH for one year after receiving reimbursement.

  • • Up to $25.00 reimbursed to employees.
